Add instructions to notification card for issues nearing expiration #6042

Description

Nearing the end of the SLA, I was spammed about issue microsoft/botbuilder-js#2781, and then continued to be spammed after replying to the issue. After the 3rd ping-post reply I realized it might be because I had neglected to add the customer-replied-to label.

I hope that by adding the label, I won't receive another ping in ~21 minutes 😄

The card should include instructions indicating that the issue's labels needs to be updated or that the bot will continue to ping the assignee. Preferably with the instructions mentioned in the README.md.
