You can reproduce the problem on Wordpress 4.0.1 and the latest Firefox version of 33.1.

Upon loading the post editor in Text mode, you can use Alt+Shift+[shortcut key] to highlight the quicktag you want to use and press enter to apply it into your post.

But if you write an article in the post long enough that you need to scroll down, you are unable to use Alt+Shift+[shortcut key] anymore.

Hi @chrischee18, thanks for the bug report.

In 4.1 the accesskey attributes were removed from the Quicktag buttons. The problem is they are quite buggy in most browsers, see #29358. They have accessibility problems. Also there are two instances of Quicktags on the Edit Post screen and having two buttons with the same accesskey fails badly.
