Zusammenfassung
Die thyreotoxische Krise ist ein Krankheitsbild, welches neben den Zeichen der Hyperthyreose durch Fieber, Herzbeteiligung (Tachykardien, Arrhythmien, Herzinsuffizienz) und ZNS-Veränderungen wie Agitiertheit, mentale Störungen bis zum Koma charakterisiert ist. Zusätzliche Erkrankungen erschweren die Diagnostik und überlagern die Symptome der ursächlich zur Krise führenden Schilddrüsenerkrankung. Unerkannt wird die Letalität der thyreotoxischen Krise auf über 90% geschätzt. Die Diagnosestellung richtet sich ausschließlich nach den klinischen Befunden. Daher ist die Kenntnis typischer und atypischer Symptome und diagnostischer Kriterien der thyreotoxischen Krise unabdingbare Voraussetzung für die Erkennung des Krankheitsbilds und die Einleitung einer entsprechenden Therapie. Da es sich um eine endokrinologische Notfallsituation handelt, sollte in jedem Fall ein endokrinologisch erfahrener Internist in die Entscheidungen bezüglich Diagnose und Therapie einbezogen werden.
Abstract
Besides the typical and in this case severe signs of hyperthyroidism the thyrotoxic crisis is characterized by additional signs and symptoms such as fever, cardiac involvement (tachycardia, arrhythmia, heart failure) and central nervous impairment eventually leading to coma. Additional diseases and comorbidities impair the diagnostic process and may mask the symptoms of thyrotoxicosis. If undiagnosed, this situation harbors a mortality of approximately 90%. The precise knowledge of typical (and atypical) symptoms is mandatory in order to rapidly recognize this situation and to initiate pharmacological treatment and/or surgery. An experienced endocrinologist should always be involved in this decision process.
